Forms Required for Sports Participation
Prior to a student being allowed to try out for a sports team, the following forms must be REVIEWED by the nurse. All forms are to be turned in to the nurse no later than 8am the day before tryouts start.
Sports candidate Physical Examination…. State law requires that any student trying out for an interscholastic sports team must have a physical within twelve months of the start of a sports season. This form must be signed and stamped by the physician. If a student has had a significant illness or injury since the exam, an updated clearance by the physician is required.
Sports Candidate Health History….A form must be completed by the parent at the start of each season. This form will serve as an update to the health information on file. The form also gives parental permission to participate in a sport.
Multi-copy Emergency contact card… This form is completed by the parent, collected at the start of each school year and kept on file in the Health office. It is used in the event of illness or injury during the school day. A copy is given to the coach to allow for parental contact in an emergency. If copies are not legible, a new form will be required.
