Registration Forms

All incoming public or non-public school students who did not attend one of our feeder districts (Bellmore, Merrick, North Bellmore or North Merrick UFSDs) immediately prior to enrolling in our district or requesting non-public school transportation/books must register.

 

The necessary forms can be downloaded from the list in the blue area to the left of this page.  A packet is available in the Central Administration offices reception room (the first room on the left at the south entrance), 1260 Meadowbrook Road, North Merrick, New York.  Any district school can print out the registration forms for you.  Each form lists the acceptable proofs/documents. 

 

ALL REGISTRATION IS BY APPOINTMENT ONLY

 

Registration is a two-part process that begins with proving residency with the Central Registrar.  After completing the applicable forms, call 992-1003 to schedule an appointment with the Central Registrar.  Appointments are given between the hours of 8:30 AM through 11:00 AM and 12:30 PM through 2:30 PM, Monday through Friday, excluding holidays, at the Central Administration offices.  The registering parent/guardian must present official photographic identification (driver’s license, passport).  After meeting with the Central Registrar, you may then schedule an appointment with the school’s Guidance Department. 

 

REQUIREMENTS FOR ALL REGISTRATIONS

 

If you are a home owner, you must submit a notarized Affidavit of Residency – Form A and proof of ownership (deed, paid tax bill, mortgage statement) and two (2) recent and different utility bills (gas, electric, water, oil). 

 

If you are a renter and have a lease/rental agreement, you must submit a notarized Affidavit of Residency – Form A, a notarized Renter’s/Non-Owner Affidavit – Form C with the lease/rental agreement and two (2) recent and different utility bills in your name at that address, and a notarized Affidavit of Landlord – Form B with proof of ownership. 

 

If you are a renter without a lease/rental agreement (i.e. living with relatives), you must submit a notarized Affidavit of Residency – Form A, notarized Renter’s/Non-Owner Affidavit – Form C with two (2) recent and different utility bills in your name at that address, a notarized Affidavit of Landlord – Form B with proof of ownership and a notarized Affidavit of Owner/Resident for the Non-Rental Resident – Form D.

 

 

If you are in the process of purchasing a home within the district, you may submit a signed Intent to Purchase Home – Form F with your purchase contract with a closing date within 30 days.  You are required to submit a notarized Affidavit of Residency – Form A with the accompanying proofs/documents 30 days following the purchase or your child will be withdrawn.

In addition to the notarized Affidavit of Residency – Form A and any other applicable residency forms, parents/guardians must submit a signed Registration Application with official proof of age for each child (birth certificate or passport) and a signed Student Residency Questionnaire (required by NYS Law) for each child. 

 

If you are the natural parent and are divorced, you must submit custody papers.

 

If you are the legal, court appointed guardian, you must submit the Court Order.

 

If you are a person acting in a non-parental relationship, you must submit a notarized Parent Affidavit – Form E1* as well as a notarized Guardian Affidavit – Form E2*.

 

If you are a Foster parent, you must submit documentation of Foster Care placement, forms BSW-241 or DSS-2999, as well as Parent Affidavit – Form E1*.

 

 

NON-PUBLIC SCHOOL

 

 

In addition to the notarized Affidavit of Residency – Form A and any other applicable residency forms, parents/guardians must submit a signed Transportation Application for Non-Public Schools to receive transportation and/or books.

 

If you are the natural parent and are divorced, you must submit court custody papers.

 

If you are the legal, court appointed guardian, you must submit the Court Order.

 

If you are a person acting in a non-parental relationship, you must submit a notarized Parent Affidavit – Form E1* as well as a notarized Guardian Affidavit – Form E2*.

 

If you are a Foster parent, you must submit documentation of Foster Care Placement, forms BSW-241 or DSS-2999, as well as Parent Affidavit – Form E1*.

 

 

Parents are responsible for providing Change of Address information to non-public schools

 

 

 

*Administrator/attorney review of Forms E1 and E2 may be required and will delay the registration process.
