Registration Forms

All incoming students who did not previously attend one or our feeder districts (Bellmore UFSD, Merrick UFSD, North Merrick UFSD or North Bellmore UFSD) immediately prior to enrolling in our district must register at our Central Registration office located at the Central High School District offices at 1260 Meadowbrook Road in Merrick.

After all your paperwork is complete and required documents are notarized, please call 992-1003 to schedule an appointment during the hours of 8:00 a.m. through 11:00 a.m. and 12:30 p.m. through 2:30 p.m. Monday through Friday, to come in and meet with our Central Registrar.

The necessary forms can be downloaded below. You may also pick up a packet in our reception room, the first room on your right after entering the south entrance of our Meadowbrook Road office.

The forms are as follows:

Registration Application- one copy must be filled out for each student you are registering.

When registering, you must present official photo identification for yourself (driver's license, passport) and bring official proof of age for each of your children (Birth Certificate with Raised Seal or Passport).

Affidavit of Residency - Form A. Every parent or guardian must fill this form out and have it notarized.

In addition,

If you are the natural parent and are divorced, you must also submit Court Custody papers.

If you are the legal guardian, you must submit the Court Order.If you are a person acting in a non-parental relationship, you must submit Form E2 - Guardian Affidavit and have it notarized and have the parent submit Form E1 - Parent Affidavit and have that notarized as well.

If you are a Foster Parent - you must submit documentation of Foster Care Placement, forms BSW-241 or DSS-2999 along with the above mentioned Form E - Non Parental Affidavit.

Residency Forms:

If you are a homeowner, you must submit proof of ownership.  This includes either an original deed, mortgage statement or recent Nassau County paid tax bill.  Additionally, you must submit two recent utility bills (electricity, gas, water, oil or cable).

If you are a renter and have a lease or rental agreement, you must submit either original.  You must also submit an Affidavit of Landlord - Form B.  With that form, you must get a copy of the Landlord's deed, recent mortgage statement or recent paid tax bill.  You must also submit a Renter's/Non-Owner Affidavit - Form CBoth Forms B and C must be notarized. Additionally, you must submit two recent utility bills (electricity, gas, water, oil or cable).

If your living arrangement is not governed by a lease or rental agreement, you must submit an Affidavit of Owner/Resident for the Non-Rental Resident - Form D and a Renter's/Non-Owner Affidavit - Form C.  With those forms, you must also get a copy of the owner's deed, recent mortgage statement or recent paid tax bill.  Additionally, you must submit two recent utility bills (electricity, gas, water, oil or cable).

If you are in the process of purchasing a home within the Bellmore-Merrick Central High School District, you must submit Form F - Intent to Purchase Home along with a copy of your purchase contract.  We will give you 30 days from your closing date to submit the proper Homeowner paperwork and will allow your children to attend school in the interim.
