Below you will find links to the health forms you may need throughout the school year. Please feel free to click on the necessary link and print out a copy of the form. The forms should be returned to the school nurse.
State law requires physical exams for all new entrants into kindergarten, and in grades 2, 4, 7 and 10. Exams must be done no more than 12 months prior to the start of school. Authorization for Administration of Medication in School Enables your child to receive prescribed medication during the course of the school day Permission for School Physical Allows the school doctor to examine your child when the doctor is available at the school
